1) Print and read ALL THREE (3) membership documents. 2) Sign and date ALL THREE (3) membership documents. 3) Send ALL THREE (3) membership documents & $10 annual dues to the club officers via one of the following methods: - Fax your documents to 866-284-6282 AND send $10 via PayPal ELECTRONIC CHECK ONLY to admin@healthyfoodchoices.org.
- Mail your documents and $10 dues (check or money order) to Healthy Food Choices Club, c/o Alison Tyler at 4315 Global Court, Norcross GA 30093.
All completed documents are to be provided to Club Officers for verification and activation of membership. |
